Use the records request form to have the following information sent directly to Open Window School by January 31, 2008 :
  • Grades/transcripts from the current year and the previous 2 years (if available)
  • Standardized test results (applicants to grades 3-5)
  • Teacher Evaluation Form: Give to current teacher to complete and return directly to Open Window School. Form for Kindergarten applicants may be downloaded here. Form for Grade 1 to 5 applicants may be downloaded here.
