How to Apply

Use the records request form to have the following information sent directly to Open Window School by January 19, 2012:
  • Grades/school records from the current year and the previous 2 years (if available)
  • Standardized test results (applicants to grades 3-8)

Current teacher evaluations are very helpful to us in learning about your child.  Please give the appropriate teacher questionnaire to your child's current teacher to complete and return directly to Open Window School. Kindergarten applicants use the PNAIS Common Kindergarten Evaluation Form and applicants to grades 1-4, use the OWS Teacher Evaluation.
